WAIVER AND RELEASE OF LIABILITY

In consideration of being allowed to participate in any way in A-F ACADEMY LTD.'s Athletic/Sports Program, related events, and activities, the undersigned acknowledges, appreciates, and agrees that:

There is a risk of injury inherent in the activities involved in this program, including the possibility of severe injury and permanent disability. While particular rules, equipment, and personal discipline may mitigate this risk, it is important to recognize that the risk of serious injury does exist.

I willingly and knowingly assume all such risks, both those that are known and those that may be unforeseen, even if they arise from the negligence of the Releasees or others. I also accept full responsibility for my participation.

I agree to adhere to the stated terms and conditions for participation. However, should I notice any unusual or significant hazard during my involvement, I commit to removing myself from participation and promptly informing the nearest official.

On behalf of myself and my heirs, assigns, personal representatives, and next of kin, I hereby release and hold harmless A-F ACADEMY LTD., its officers, officials, agents, employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used for the event (“Releasees”), from any and all injury, disability, or loss or damage to person or property, whether caused by the negligence of the Releasees or otherwise.

I have carefully read this Release of Liability and Assumption of Risk Agreement, fully understand its terms, and acknowledge that by signing it, I am relinquishing substantial rights. I sign this agreement freely and voluntarily, without any form of inducement.
