A-F ACADEMY LTD. – PARTICIPATION AGREEMENT & RELEASE

I understand that participating in A-F ACADEMY LTD.’s athletic and sports programs involves physical activity and carries a risk of injury, including serious injury. While the organization takes precautions, accidents can happen.

By signing below:

1. I accept all risks of participation, including those that may arise from the actions or negligence of others.
2. I agree to follow the rules, instructions, and safety guidelines. If I notice anything unsafe, I will stop and notify an instructor.
3. On behalf of myself (and my heirs, representatives, and next of kin), I release A-F ACADEMY LTD., its staff, volunteers, sponsors, and property owners from any claims related to injury, loss, or damage that may occur during participation.
